PATCH originated with a
partnership between the Centers for Disease Control (CDC), State health
agencies, and local communities.
In Gage County it consists of members representing schools, county extension, public health, library, fire and police depts., chamber of commerce, hospital, community organizations, churches, etc. The PATCH community, under the guidance of a local coordinator, establishes a community group and steering committee to carry out PATCH activities. The process yields not only improvements in community health status but also a functioning coalition with the capacity to conduct community-based health promotion.
